Partnering For Impact
If your organization does good works for the sake of our Wells County residents that makes an impact toward helping our community thrive, then please apply for funding below. We are looking forward to a partnership, not just being a funding agent for you. We expect a working relationship that is reciprocal and mutually beneficial. We will only be better together.
As a funded agent, you are also expected to participate in future community Funded Partner Meetings, participate in our annual campaign by helping our campaign chair/Executive Director present during our 2025/2026 campaigns across Wells County in the Fall of 2025 & 2026. We also require quarterly updates be submitted through Jotform on these dates: July 15, October 15, January 15 and April 15 if funding is approved.
Applications and all documents must be submitted by April 1st, 2025, in order to be considered for funding for 2025-26 funding cycle.
Required documents are as follows:
- Agency Funding Form which contains Program Funding Requests
- Budgets for both Agency and each program submission – be sure to include all requested details.
The following documents should be uploaded before hitting submit.
- Current By-Laws and Mission Statement
- Current board list with names, addresses, when their term expires (where applicable), and a list of board officers
- 501(c)(3) determination letter
- Most current audit and form 990 (Annual Audits are strongly suggested but agency may submit current financial position statement as well as annual budget and spending reports when audit not available for agencies with annual income of $300K or less)
- Agency Conflict of Interest policy
- Agency policy on Diversity, Equity and Inclusion, including Non-Discrimination
- Success story for EACH program submitted. Story should have occurred in the last 12 months (first names only). Include how the program benefitted the client.
- Agency and Program budgets using excel spreadsheet provided in initial email.
- Memorandum of Understanding and Anti-Terrorism certifications provided in initial email.
United Way of Wells County will partner with and support projects that make an impact in Wells County within one of their identified vision council impact areas. Grants will be made for one-time funding requests and are not intended for ongoing program expenses.
